1. Voluntary Participation & Assumption of Risk

I acknowledge that martial arts training and athletic physical activity involve inherent risks including, but not limited to, physical exertion, contact, minor and major physical injury. I affirm that the participating student/guest is physically fit and has no medical condition that prevents safe participation in beginner karate training.

2. Medical Treatment Authorization

In the event of an emergency, I authorize Super Kick Karate instructors and staff to secure necessary medical care, first aid, or transport on behalf of the participant if the parent/guardian cannot be immediately reached.

3. Release of Liability & Dojo Rules

I hereby release and hold harmless Super Kick Karate, its owners, instructors, and affiliates from all claims, liability, or demands resulting from participation in trial sessions, buddy passes, or studio events under standard operating guidelines.
